“Heart attack on a bun” (“Cholesterol on a bun”)
“Heart attack on a bun” (or “cholesterol on a bun”) is a dish loaded with potentially artery-clogging food. “Heart attack on a bun” is cited in print since at least February 1992 and “cholesterol on a bun” since August 2000. The expression “heart attack on a bun” first described hot dogs and hamburgers, but was gradually extended to describe other meals.
Similar expressions include “heart attack on a plate” (since April 1988), “heart attack in a bag” (since January 1995), “heart attack on a stick” (since August 1995), “heart attack in a can” (since February 1998), “heart attack in a bottle” (since July 1999) and “heart attack on a rack” (since 2009).

Bacon or no, Wendy’s still has it beat for most lethal substance ever to grace a bun - the TRIPLE with cheese. I can feel my cholesterol level rise just thinking about it. C’mon McDonalds, give us something a bit more lethal!!! 😊
